Ready to refer a patient?
There are multiple ways to send your referrals; pick the option that is best for you! Please include patient demographics, certificate of medical necessity, chart notes, and relevant lab work.
Please note that before we can ship an order, we will need a signed Assignment of Benefits form on file in order to bill the patient’s insurance. Click here to access the online Assignment of Benefits form.
If your patient cannot complete the online form, click here to download the form for your patient to print and sign.
